All Pellets are Not Created Equal

In early 2017 we enhanced our dedicated pellet facility in Tennessee to meet the requirements of, and became, an FDA Registered 503B Outsourcing Facility. Our quality standards already surpassed what was required by USP <797> and we saw that in the near future the gold standard in our industry for pellets would be compliance with the FDA’s standards for CGMP (current good manufacturing practices), the same as large commercial drug manufacturers. Physicians now can order for office use (please check with your State Board of Pharmacy to determine if patient specific prescriptions are required.)

What are Sterile Pellets?

Sterile pellets are a small and cylindrical dosage form, about the size of a grain of rice.

Sterile pellets are used to treat hormone deficiencies and imbalances by implanting them in the subcutaneous tissue of the gluteal or abdominal regions in men and women. They are placed beneath the skin by a health care practitioner, during a simple office procedure.  This method of hormone delivery allows the body to slowly draw out small, physiologic doses of the needed hormone, providing for a more consistent steady-state delivery. The sterile pellets typically dissolve over a period of three to four months, sometimes lasting up to six months, at which time, new pellets will need to be implanted.  The simple office procedure requires a small local anesthetic injection in the insertion area, then a tiny incision is made in the skin.  A device called a trocar, is then inserted into the subcutaneous layer beneath the skin.  The needle end of the trocar is removed, the pellets are placed into the trocar device and a blunt obturator is inserted into the trocar to gentle advance the pellets down into the insertion tract.  Once pellets are placed, the practitioner will typically close the tiny incision site with a simple sterile strip closure and then cover with an adhesive bandage.

How are Sterile Pellets Created?

The most common hormones used to create pellets are estradiol (E2) and testosterone. At Wells Pharmacy, we do not use any chemical binders to prepare the pellets – only hormone powder and stearic acid are used. Testosterone USP or Estradiol USP is fused together with stearic acid in a press to form a small cylinder. Pellets are sterilized using E-Beam irradiation while in the blister pack thus ensuring both the pellet and the packaging are sterile. Our dedicated pellet facility is an FDA Registered 503B Outsourcing Facility and as such we adhere to their CGMP (current good manufacturing practices) and all USP regulations for hazardous compounding requirements.

Why use Pellet Hormone Therapy?

Pellet hormone therapy has been used since the late 1930s and is often considered one of the most natural ways to deliver hormones to the body. The hormones are identical to the body’s own both structurally and biologically. Because the body does the work on its own once the sterile pellet is inserted, this increases compliance because patients do not have to remember to take a pill, give an injection, or apply a cream each day.
